* feat(skills): close the delivery loop - tdd discipline, debugger seam findings, refit bite-proof, two-axis review
Four loop edges that left the post-delivery circuit open:
- tdd: new model-invoked skill defining the discipline launch Phase 4
references ("the tdd discipline at the seams approved in C2") - one
test, one implementation, expected values from an independent source
of truth, substitutes only at the seam's declared boundary class,
refactoring kept out of the loop; keyword tdd-mode now points at it;
registered in the plugin manifest, workflow registry, and skill lists
(the tdd -> test-engineer agent shortcut retires with it).
- debugger: before testing any hypothesis, rank 3-5 candidates by
evidence fit, then still test one at a time; tag temporary debug
output [DEBUG-<4hex>]; redact secrets before logs leave the session;
a missing seam for a regression test is itself a finding handed to
architecture-survey/refit; record the confirmed hypothesis in the
fix's commit message where conventions allow.
- refit: a newly installed check must be proven to bite before it
counts as landed - run it clean, demonstrate it failing on a planted
violation, revert the violation.
- omc-review: substantial diffs are reviewed as two parallel sub-agent
passes (standards axis, intent axis), reported separately, never
cross-ranked; small diffs stay one pass.
