OMA Maintainer Bot is a repository-local, evidence-first path from a narrowly

The host may then acknowledge the result as `DRAFT_PR_CREATED`; that state is
never inferred from a proposal.
The workflow handles only an `issues.labeled` delivery whose exact label is
`agent-ready`. It never scans or develops all open issues, does not use
`pull_request_target`, and does not depend on a maintainer's local computer or
a long-running server.
## GitHub Actions activation flow
1. The default-branch workflow publishes or updates one BOT status comment with
`STARTED`, Actions run URL/ID, and a freshly resolved base SHA. If an older
claim exists, the visible STARTED notice does not replace that machine claim
before duplicate/stale checks finish.
2. GitHub serializes runs by repository and Issue number. The job uses a
GitHub-hosted ephemeral runner, a 45-minute timeout, pinned Node/npm
versions, and explicit `actions: read`, `contents: write`, `issues: write`,
and `pull-requests: write` permissions.
3. The host refetches repository, Issue, labels, material comments, timeline,
actor permission, current default-branch SHA, trusted setting attestation,
existing BOT state, branch, and PR metadata. It performs admission before a
model call or repository edit.
4. A separate child receives `DEEPSEEK_API_KEY` plus a small non-secret
environment. It receives no GitHub, npm, Actions runtime, App, SSH, or other
write credential.
5. If OMA reaches `DRAFT_PR_PROPOSAL_READY`, a new deterministic host process
refetches all authorization facts and calls `revalidateDraftPrSafeOutput()`
against the actual worktree before any branch, commit, push, or PR call.
6. The writer stages exactly the reviewed files, rejects extra/untracked/
deleted/renamed/symlinked/out-of-scope content and unsafe local Git hook,
proxy, credential, include, filter, or URL-rewrite configuration. Writer Git
processes disable hooks plus global/system credential configuration, use a
deterministic branch and canonical HTTPS destination, and create at most one
Draft PR. Every terminal path updates the single status comment and Actions
summary.
GitHub only triggers an `issues` workflow when its workflow file exists on the
default branch. Repository Actions settings must also enable “Allow GitHub
Actions to create and approve pull requests”; activation v1 uses only the
creation half and never approves. See GitHub's
[trigger documentation](https://docs.github.com/en/actions/how-tos/write-workflows/choose-when-workflows-run/trigger-a-workflow)
and [repository Actions settings](https://docs.github.com/en/repositories/managing-your-repositorys-settings-and-features/enabling-features-for-your-repository/managing-github-actions-settings-for-a-repository?apiVersion=2022-11-28).

### Crash and stale lease semantics
### Cross-run authority, crash, and stale semantics
The file state store is an authoritative local idempotency and audit ledger;
it is **not authoritative crash recovery** and does not resume model
conversations or edits after process death. There is no lease renewal or
fencing-based automatic takeover in this MVP.
`FileRunStateStore` is authoritative only inside one live engine/writer handoff.
Because GitHub-hosted runners are ephemeral, `$RUNNER_TEMP` is never the
cross-run authority. Activation v1 instead uses one identity-checked
`github-actions[bot]` status comment containing a bounded machine-readable
claim ledger, Actions run status, and deterministic branch/PR metadata. A user
comment containing the same marker is ignored. The ledger retains prior
runKeys when a later revision becomes visible and fails closed at 64 claims
instead of silently dropping idempotency history.
An active lease blocks a concurrent claim. If a `RUNNING` record loses or
exceeds its lease, a new claim does not resume it: the old record is atomically
marked `NEEDS_HUMAN` with an auditable stale reason. `failStaleRun()` is the
controlled local entry for a maintainer or outer control plane to make that
transition explicitly; it requires the exact run key/run ID and refuses an
active lease. Recovery means inspecting the isolated worktree, selecting a new
clean base or material issue revision, and issuing fresh maintainer
authorization. The same revision/base pair remains terminal and cannot be
silently retried. Recovery does not mean replaying a checkpoint.
GitHub concurrency prevents two workflow jobs from editing the same Issue at
once. A repeated terminal runKey is a duplicate and does not invoke the model,
push, or create another PR. A lost `RUNNING` claim becomes `NEEDS_HUMAN`; the
fallback preserves its revision/runKey and automatic model-conversation resume
is forbidden. Recovery means inspecting the Actions run, ensuring the prior
work is not active, selecting a clean base or material Issue revision, removing
and reapplying authorization deliberately, and starting from a fresh model
conversation. Local checkpoints are not recovery authority.

## Verification and activation gaps
## Status and operations
Unit/integration tests use mock OMA adapters and do not need API keys. The
fixture dry-run is read-only. A live DeepSeek canary is intentionally not part
of default tests and every activation record must state whether it ran. For the
MVP acceptance run, a credential-isolated, single-file fixture completed
`AGENT_READY` through edit, deterministic validation, fresh approval, proposal
creation, and the actual-worktree safe-output gate. The canary passed two
registered validations and all three acceptance criteria; it performed no
GitHub write. A live GitHub canary must use a disposable repository or
supervised Draft PR and requires separate authorization; this package itself
still must not receive the write token.
The Issue's single BOT comment and Actions job summary use these public states:
- `STARTED`: the label delivery is visible and deterministic checks are starting.
- `RUNNING`: fixed revision/base, permission, DoR, scope, duplicate, and setting-attestation checks passed; the isolated engine is running.
- `NEEDS_CLARIFICATION`: the strict Issue template or Definition of Ready is incomplete or conflicting.
- `MANUAL_ONLY`: policy classifies the work as architecture, security, permissions, privacy, license, CI/release, broad API/refactor, uncontrolled dependency, or otherwise human-only.
- `NEEDS_HUMAN`: stale/crashed state, drift, setting, conflicting branch/PR, or safety gate requires intervention.
- `FAILED`: infrastructure or engine failure produced no eligible Draft PR.
- `DRAFT_PR_CREATED`: exactly one open Draft PR exists; human review remains required.
To configure activation without exposing a value:
1. Add or rotate an Actions repository secret named `DEEPSEEK_API_KEY` under
**Settings → Secrets and variables → Actions**. Never put the value in an
Issue, workflow, repository file, log, artifact, or comment.
2. Under **Settings → Actions → General → Workflow permissions**, enable the
repository option allowing GitHub Actions to create pull requests. Do not
enable or change it as a side effect of a code deployment.
3. After an administrator verifies that setting, add a repository Actions
variable named `OMA_MAINTAINER_BOT_PR_CREATION_ENABLED` with the exact value
`true`. The host checks this trusted non-secret attestation before model
execution and again before writing; a missing, empty, or different value
fails closed. Remove or change the variable before disabling the setting.
4. Keep the workflow's explicit permissions and branch protections under human
review. The default workflow permission may remain read-only because this
workflow declares its required scopes explicitly.
The REST endpoint that reads this repository setting requires repository
`Administration: read`, which workflow `GITHUB_TOKEN` cannot request. v1
therefore does not introduce an administration-capable token merely to inspect
the setting. The operator attestation is the minimum safe pre-model gate; the
actual Draft PR API remains the final capability check. See GitHub's
[Actions permissions API](https://docs.github.com/en/rest/actions/permissions?apiVersion=2022-11-28).
At the implementation audit on 2026-08-11, the repository API reported
`default_workflow_permissions: read` and PR creation disabled. No setting or
secret was changed by implementation work. The explicit workflow permissions
are compatible with the read default, but disabled PR creation is a deliberate
activation blocker until a separately authorized operator step. Secret presence
and the attestation variable were not inspected or configured.
To disable the bot, disable the workflow in GitHub Actions for an immediate
operational stop, or merge a trusted policy change setting `enabled` to
`false`. The latter may still allow the trigger to publish a disabled terminal
status, but never runs the model or writer. For key rotation, replace the
Actions secret, verify no run is active, and revoke the old provider key; runs
never persist the key in state or artifacts.
`GITHUB_TOKEN`-authenticated writes do not recursively trigger most new
workflow runs. Therefore a Draft PR created by v1 should not be assumed to have
automatically started the repository's ordinary `pull_request` CI; the trusted
pre-PR validation registry is mandatory, and maintainers must inspect the PR's
actual checks. This recursion behavior is documented in GitHub's
[`GITHUB_TOKEN` security reference](https://docs.github.com/en/actions/concepts/security/github_token).
## Verification and first live canary
Unit/integration tests use a mocked GitHub client and scripted OMA adapters and
need no key. The synthetic #488-style path proves that
`packages/create-oma-app/tests/runtime.test.ts` becomes an exact one-file scope,
runs both `OMA_MODEL=ambient-model` and unset focused tests, and receives the
trusted create-oma-app lint/test/template checks. Default tests make no GitHub
write and no live provider call.
A first live canary is a separate, explicitly authorized operation:
1. Confirm Issue #488 is open and `agent-ready` is absent; inspect its material
revision rather than reusing an old authorization.
2. Configure/verify the DeepSeek secret, repository PR-creation setting, and
exact attestation variable.
3. Verify the workflow exists on `main`, its action/runtime pins and production
policy are reviewed, and no conflicting BOT branch/PR/run exists.
4. Reapply `agent-ready` once with a write/maintain/admin actor and observe
STARTED/RUNNING plus run URL, ID, revision, and base SHA.
5. Verify only `packages/create-oma-app/tests/runtime.test.ts` changed, every
registered validation passed without truncation, and exactly one Draft PR
was created and linked.
6. Repeating the same authorization must reuse the terminal runKey/PR and must
not create a second branch, push, or PR.
7. Confirm no Ready, approval, merge, close, release, publish, tag, or deploy
action occurred.
Do not perform these steps during ordinary local implementation or testing.

# OMA Maintainer Host
This private workspace is the deterministic GitHub Actions activation and
Draft PR writer for `@open-multi-agent/maintainer-bot`. The OMA engine remains
credential-free with respect to GitHub and npm; this host launches it in a
separate allowlisted environment and obtains the GitHub token only in distinct
prepare/finalize processes.
The GitHub-native activation entry point is
[`.github/workflows/maintainer-bot.yml`](../../.github/workflows/maintainer-bot.yml).
It handles only an exact `issues.labeled` / `agent-ready` event, checks out the
fixed default-branch SHA without persisted credentials, runs the OMA engine in
a secret-minimized child, and invokes the writer only after final safe-output
revalidation. The host never marks a PR ready, approves, merges, closes,
releases, publishes, tags, or deploys.
Eligible production runs also require the trusted repository variable
`OMA_MAINTAINER_BOT_PR_CREATION_ENABLED=true`, set only after an administrator
verifies the corresponding Actions repository setting. The variable is an
attestation, not a credential; absence stops before model execution.
The production policy is
[`config/production-policy.json`](config/production-policy.json). It is not an
Issue-controlled config and is separate from the maintainer-bot fixture
configuration. Operational behavior and activation steps are documented in
[`docs/maintainer-bot.md`](../../docs/maintainer-bot.md).
All host tests use a fake GitHub implementation and scripted OMA adapter. They
perform no network write and require neither `GITHUB_TOKEN` nor
`DEEPSEEK_API_KEY`.

npm run lint -w @open-multi-agent/maintainer-host
npm run test -w @open-multi-agent/maintainer-host

OMA Maintainer Bot used remote DeepSeek inference to propose this bounded change. Repository checkout, editing, validation, safe-output revalidation, commit, push, and Draft PR creation were controlled by deterministic GitHub Actions host code. No GitHub credential was exposed to the model process.
This pull request is intentionally Draft. The bot did not approve, merge, close the Issue, release, publish, tag, or deploy.

\`packages/create-oma-app/tests/runtime.test.ts\` reads an ambient OMA_MODEL and fails for unrelated reasons.
## To Reproduce
Run the focused runtime test with OMA_MODEL set and observe two deterministic failures.
## Expected behavior
The runtime tests fully control and restore OMA_MODEL for every test.
## Acceptance criteria
- The focused runtime test passes with OMA_MODEL=ambient-model.
- The focused runtime test passes without an ambient OMA_MODEL.
## Target paths
- \`packages/create-oma-app/tests/runtime.test.ts\`
## Out of scope
- Production runtime behavior, dependencies, CI, release, public APIs, and other workspaces.
