Add Command-R+ 104B Q4_K_M results: 128K full native context on M5 Max,
PPL 4.024, symmetric turbo3 +3.6%. Document macOS GPU memory wall root
cause and fix (iogpu.wired_limit_mb). Update abstract, comparison table,
and limitations.
