Mihawk 1bc79fbf2f Make the sampler fallback patch idempotent

Regression test failed on 0023 because its marker was absent from its own replacement; it passes after the marker correction. Regenerated configured shader source removes accumulated copies.

Verify: bash tools/verify.sh (all five gates, 22 tests). Evidence: evidence/frontend-patch-idempotence/{capture,expectation}.json records unchanged SHA256 values after full patch reapplication; replay with tools/verify.sh evidence. Host tooling only; no new GPU acceptance claim.
