Defer invocation foreign-key checks until session cascades finish. Bound migration lock waits and cover deletion and migration behavior.

refactor(agents): simplify response storage and checkpoint metadata

Rename stored response rows to items and derive replay order from steps and tool calls. Remove duplicate metadata and obsolete checkpoint aliases, and align the development migration and tests.
Handle cancellation callbacks consistently and preserve failure tracing across LLM generation paths.
